Dental
The dental plan, administered by Delta Dental, covers three types of services: preventive care, basic services, and major services.
- Preventive care stops dental problems before they start. The Plan covers two teeth cleanings and exams each year at 100%.
- Basic care is common dental work like fillings, root canals, and gum treatments.
- Major care is more involved dental work, such as prosthodontics and bridges.
Your $900 annual maximum for dental coverage applies to in-network care, $750 for out of network care.

Vision
The Plan provides you and your covered family members vision coverage through an arrangement with Vision Service Plan (VSP). You receive discounted services and a higher level of coverage when you use a VSP provider.

Life, AD&D, and Weekly Disability Benefits
Income protection for your family is essential in case something serious happens to you. Life, AD&D, and short-term disability coverage administered by Metropolitan Life Insurance (MetLife) protects your family’s financial security in those situations.
Weekly Disability Benefits
The Plan pays you a weekly disability benefit if, while you are covered, you become totally disabled due to a non-occupational sickness or injury.
If eligible, you receive a weekly disability benefit equal to $400 per event, for up to 26 weeks (less mandatory Social Security withholding). Your weekly disability benefit is offset by any disability benefit you receive from Social Security, as well as any disability pension benefit you are eligible to receive. If the total weekly value of your Social Security and disability pension benefit(s) exceeds the weekly disability benefits you are eligible to receive from this Plan, then the Plan does not pay you a weekly disability benefit.
